5. 5. Your Media Outlet / Organization's Website URL
6. 6. The Type of Request You are Making
7. 7. Your Desired Response Date or Your Deadline
8. 8. The Subject Line of Your Media Inquiry
9. 9. The Names of the Individuals You Are Directing this Media Inquiry To
10. 10. Your Message / Details of Your Request
